Associate Provider

Community Carers is a registered nurse-led, clinically focused associate provider supporting older adults and aged care providers across the Central Coast.

We partner with a range of Support at Home providers to deliver safe, coordinated, and high-quality care, aligned with the Aged Care Quality Standards and supported by clinical oversight, education, and ongoing review to ensure services remain appropriate as needs change.

We work across multiple aged care pathways, including Support at Home, Commonwealth Home Support Programme (CHSP), and residential aged care, providing flexible and integrated support.

A nurse-led approach ensures care is clinically guided, coordinated, and responsive — so support feels clear and manageable.

Support at Home and CHSP Services

Community Carers provides registered nurse-led support at home services on the Central Coast, working in partnership with approved providers to deliver safe, coordinated, and high-quality aged care support.

While Community Carers is not a CHSP approved provider, we work closely with local Commonwealth Home Support Programme (CHSP) providers to help clients access the services they need, supporting care coordination, clinical oversight, and aged care navigation.

Our approach includes clinical assessment, care planning, education, and ongoing review, ensuring care is aligned with the Aged Care Quality Standards and remains responsive as needs change.

We maintain strong quality and compliance systems, ensuring all clinician requirements — including criminal record checks, insurances, and vaccinations — are current, while our team is supported to continually upskill to meet the evolving needs of clients.

Through strong partnerships, clinical oversight, and robust compliance systems, we help ensure care is connected, compliant, and responsive — supporting older adults to live safely and confidently at home.

Residential Aged Care Services

Community Carers has experience supporting residential aged care providers on the Central Coast, delivering registered nurse-led clinical services to residents within aged care facilities.

We partner with providers to deliver consistent, well-coordinated care, ensuring continuity through reliable scheduling and a stable clinical team supporting residents over time.

Our RN-led approach focuses on clinical assessment, care planning, medication management, wound care, continence and skin integrity, alongside education and support for care staff.

We are experienced in working within AN-ACC funding models and the Aged Care Quality Standards, ensuring services are delivered in a structured, compliant, and evidence-based manner.

What is the difference between Support at Home and CHSP?

The Commonwealth Home Support Programme (CHSP) provides entry-level support for people with lower care needs, while Support at Home (formerly Home Care Packages) is designed for people with more complex or ongoing care needs

Do I need to be approved to receive services?

Yes. Most government-funded aged care services require an assessment through My Aged Care, where your needs are assessed and the appropriate level of support is determined.

Yes — Community Carers provides aged care navigation and care planning support, helping you understand your options, access services, and ensure your care is coordinated.
